Hello Please note I have removed the download button to avoid PDF being downloaded however it still shows in the mobile view how can I fix that.
Also is it possible to place a shortcode in PDF for an opt-in form to replace standard download?

You need to disable it for narrow mode as well: WP Dashboard->3D FlipBook->Settings->Actions->cmdSave(-enabled, -enabledInNarrow).
No.

You need to use css, html and java script. Shortcode does not work there.

CSS layer can help you: https://www.youtube.com/watch?v=ekpAGfmPI5k , it allows to add any content in a flipbook as the second layer.
